What a TMV does, and why it fails

A thermostatic mixing valve sits between the 60 °C hot supply and the cold supply, blending them to deliver a safer outlet temperature, typically 45 °C: at basins, showers and baths in commercial settings where users can't reliably regulate the tap themselves. Healthcare, aged care, early childhood centres, schools and disability accommodation are all required to use TMVs at sanitary fixtures under AS/NZS 3500.4.

Inside the valve body is a wax thermostat cartridge that expands and contracts as temperature changes, sliding a piston to balance the inlet ports. The cartridge is the part that fails, usually because:

  • Sediment from the hot supply line accumulates around the cartridge and slows its response
  • Mineral scale in hard-water areas builds up on the piston and seats
  • Wax fatigue: after thousands of expansion/contraction cycles, the wax loses some of its responsiveness
  • Internal seal degradation: O-rings perish over time, particularly at higher inlet temperatures

These are normal, predictable wear modes. That's why TMVs are designed for annual service rather than fit-and-forget.

The three main failure symptoms

1. Temperature creep (outlet running hotter than set-point)

This is the dangerous one. The valve's wax thermostat slows down or sticks slightly open on the hot inlet, and the outlet drifts above 45 °C. At 50 °C, water can cause a third-degree burn in about five minutes on adult skin. At 55 °C it's about 30 seconds. At 60 °C it's about 5 seconds.

In an aged care setting, a resident with reduced sensation may not feel the water getting hotter until the burn is well underway.

How to spot it before the next annual service:

  • Staff or residents commenting that the basin water "feels hotter than usual"
  • A digital thermometer at the outlet reading above 46 °C at full hot flow
  • Fewer complaints about lukewarm water (which sounds like a good thing, but isn't, it means the cold-side blending isn't working as hard)

If you notice any of these, get the TMV tested before the scheduled annual service. Don't wait.

2. Reduced flow at the affected fixture

The cold-side inlet port can clog with sediment over time, particularly on installations downstream of older galvanised hot water pipework. The result: the valve can't pull in enough cold water to balance the hot, and the way it self-protects is by closing the hot side, which restricts overall flow.

What you see at the basin: the tap "doesn't feel like it's running at full pressure" even though every other fixture in the building is normal. The TMV upstream is the bottleneck.

This isn't a safety failure, flow restriction is the valve being conservative, but it does mean the valve is overdue for service.

3. Unstable temperature during a single use

The wax thermostat is supposed to maintain ±1-2 °C across normal supply pressure variations. When it starts to fail, you get oscillation, the outlet swings between hotter and cooler during a single hand-wash or shower.

This is usually the cartridge's response time degrading. Sediment around the piston means it can't react quickly to a momentary pressure dip when another fixture in the building is opened. The user gets a quick blast of hotter water followed by an over-correction to cooler.

In a busy ward where multiple basins draw simultaneously, this is the most common complaint we hear.

Other signs to watch for

  • Visible drip at the valve body: the cartridge O-rings have failed; cartridge replacement is required
  • Drip from the temperature adjustment cap: same diagnosis
  • Failed annual recommissioning test: outlet temperature, response-to-cold-failure, and stability tests all need to pass for the certificate to issue
  • Service interval exceeded: if your last documented service was more than 12 months ago, the valve is non-compliant regardless of its current performance
  • Audit finding: facility audits sometimes turn up TMVs that have drifted, are missing service tags, or have no recent commissioning record

What annual TMV servicing actually involves

Under AS 4032.3, each TMV in a healthcare or aged-care facility (and similar) must be tested and recommissioned every 12 months by a licensed plumber qualified to do this work. The procedure:

  1. Inspect the valve body for visible damage, leaks, and existing tag
  2. Measure outlet temperature at multiple flow rates with a calibrated thermometer
  3. Cold-supply failure test: isolate the cold inlet and confirm the hot outlet shuts off within the allowed time
  4. Test mixed-temperature stability: open another fixture and confirm temperature doesn't drift
  5. Replace the cartridge if the test fails OR if the cartridge is at end of its service life
